Continuing Education Programs Student Library Card Application

Service for

  • For NCKU students of Continuing Education Programs only.

Application

  • Department staff should submit the NCKU Library Card Request Form for Continuing Education Programs, one copy of NCKU enrollment document and one recent photo of each student to the Circulations Desk.
  • Monthly fee: NT$100 per month.
  • Lost or damage, please provide one copy of NCKU enrollment document, one recent photo and NT$ 200 processing fee for replacement.

Important Notes

  • It is necessary for the applicant to provide all required information to his/her department in advance for application.
  • Department staff should send the request form in excel format, and mark the name in the back of each photo. It takes 3 working days to finish.
  • The Card is for personal use only. All the library privileges would be suspended for one month if lend to other person.
  • Scan the library card barcode to enter.
